The nurse practitioner (NP) is an advanced practice nurse who delivers care in primary and specialist areas in a wide variety of health care settings. Previously considered an expensive healthcare option, it is now recognised as an integral component of the evolving model of primary health care. The objective was to describe the role of the NP in a hepatitis C care team model and evaluate the efficacy of this role in a PWHC programme. A retrospective descriptive study was conducted on five patients diagnosed with chronic hepatitis C (CHC) and who have accessed an evidence-based PWHC programme. Data collection included a review of medical records (using a structured form) and patients were interviewed using a standardised questionnaire. Data were extracted from medical records and validated by using interviews and patients' self-reports. Findings were reviewed with the NP team, and treatment strategies were determined. The five patients who have accessed the hepatitis C care team model have become long-term chronic care patients. Four patients were cleared of infection at the end of the study, while one patient died as a direct consequence of complications from CHC. Based on the data and information obtained from patients' interviews, NP care led to a decrease in hospital and clinic visits, and an increase in the availability of preventive services. The NP was an integral part of the PWHC programme and is successful in leading patients to an improved quality of life, a significant reduction in morbidity, and the eventual clearance of infection..
